Submit a Report
Use this form to tell us about any CMC digital resource you cannot access — a web page, PDF, video, application, or Canvas course. Reports are acknowledged within one business day.
Before you submit, please include in your description:
- The URL, or the name of the app or document (e.g. Canvas, Workday, “FY2025 Budget PDF”)
- What you were trying to do, and what stopped you
- Any assistive technology you were using, and its version — screen reader, keyboard only, voice control, or magnification
- Whether you are blocked right now, or can wait for a normal response
Please start your subject line with “Accessibility barrier:” so your report reaches the right team.
Reports are handled by CMC ITS. Your email address is required so we can acknowledge your report and tell you when the barrier is fixed. This form does not support anonymous submissions; for personal disability accommodations, see the sections below.
Other ways to report. CMC also maintains a Report a Web Accessibility Issue form, reachable from the College’s Report a Concern page under “Digital Accessibility Concerns.” Either route reaches CMC — use whichever you prefer. For personal disability accommodations, see the sections below.
What Happens After You Report
Acknowledgment within 1 business day
You will receive a confirmation that your report has been received and a ticket number for tracking.
Routed to the responsible team
StratCom handles websites and web content; ITS handles applications and platforms; Faculty and CTL handle Canvas course content.
Accessible alternative if urgent
If you are blocked, an accessible alternative will typically be provided within 3 business days while the underlying issue is resolved.
Follow-up when the fix is complete
ITS will notify you at the email address you provided once the accessibility barrier has been remediated.
Personal Disability Accommodations
This form is for digital accessibility barriers in CMC technology. Requests for individual disability accommodations are handled separately.
Students. If you need personal disability accommodations in coursework — extended time on exams, note-taking support, or accessible format requests — contact Accessibility Services, in CMC’s Office of Civil Rights: accessibilityservices@cmc.edu or (909) 607-0316.
Faculty and staff — workplace accommodations. If a workplace tool or system is not usable for you — HR self-service, payroll and tax forms such as the W-4, benefits enrollment, required trainings, or time reporting — CMC wants to know.
Requests for individual workplace accommodations are handled by the Office of Human Resources: hr@cmc.edu or (909) 621-8490. You can also report the underlying technology barrier using the form above, so ITS can assess and remediate the system itself.
